Case Study #3 - Complex Wound Care with Respiratory Failure

This 59 year old male was admitted to Banner Estrella Medical Center on 10/24/08 and was found to have a complicated wound infection that would require surgical debridement and wound vac therapy.  He developed post op respiratory failure which required mechanical ventilation and an eventual tracheotomy.  This gentleman was subsequently transferred to the Acute Care Unit at Trillium Specialty Hospital – West Valley on 11/12/08.  He received a course of IV antibiotics and wound care, as well as introduction of Trillium’s mechanical ventilation weaning protocol under the management of the Respiratory Therapy department.  The patient also received physical therapy in order to regain his strength and endurance.  On 12/1/08, this patient was successfully weaned from the ventilator and on 12/4/08, the patient was transferred to Trillium’s Transitional Care Unit where he could continue physical therapy and wound care management. The wound vac was discontinued and his wound was completely healed by 12/15/08.  After successful airway management, he no longer required his trach which was eventually removed entirely on 12/18/08.  Physical therapy continued for a few additional days resulting in this patient’s successful discharge to home on 12/24/08 to be with his family, in time for the holiday.

Summary of this case:

This patient's case was particularly complicated due to complex wound care in the presence of respiratory failure. Often, successful wound healing in a compromised patient can take an extended period of time and require careful attention. This case demonstrates how beneficial it was for this patient that Trillium had the ability to offer multiple levels of care and utilize the Transitional Care unit to provide the additional care needed for this patient's wound to heal after his respiratory problems were resolved. Trillium's staff was pleased to have achieved a complete success for this patient in time for his return to home with family, before the holiday.
