How to Refer a Patient

To refer a patient to us, please contact the Admitting/Registration Department of the Trillium location most suitable for the particular patient. You can expect a prompt response from our Admitting Department and in most cases, we will arrange direct assessment  of the referral by one of our field Clinical Liaisons
