The Discharge Process –What to Expect

Once a patient’s stay at Trillium Specialty Hospital approaches completion, discharge arrangements will be made by our Case Management staff. Ideally, for those patients who are able to return to their prior level of activity in their home setting, we will arrange any continuing services required such as home health care, and outpatient follow-up.

For those patients requiring extended services in another inpatient setting, our staff can provide community resources and assist the patient and family in making appropriate decisions and choices regarding continuing care.
