Trillium announces the launch of its Return to Acute Prevention Program ("RAPP"). This program, developed in cooperation with a selected group of insurance plans, is aimed at preventing unnecessary discharge of patients back to the acute hospital setting from the Trillium TCU/SNF level of care.

At a time of increased industry focus on preventing unnecessary hospital admissions, Trillium believes its new program will provide a safe and effective means of avoiding disrupted care and better patient outcomes.
