Transitional Care Unit
It is often the case that additional step-down care is necessary for a patient to meet their full recovery potential and achieve a safe return to home. The ability to provide on-premises step-down care is one of the features that makes Trillium superior. When a patient no longer needs the intensity of acute hospitalization, but still requires nursing care, rehabilitation and other services, the patient can remain at Trillium Specialty Hospital in the Transitional Care Unit (TCU). In doing so, the patient will transition from a higher level of care to a lower level of care, without experiencing a disruption in service, usually keeping the same physician(s), nurses, therapists and services for continuity.
Trillium Specialty Hospital’s TCU provides the patient with a unique care experience. Our nursing and rehab teams provide advanced services for sub-acute patients and can offer capabilities simply not available in conventional nursing homes, skilled nursing and sub-acute facilities. One of the many reasons for this is that Trillium's TCU has access to many of the features and services of a hospital because it is located within one.
